In answer to your question, "No" my CD has no booklet, just a blank insert with original album artwork on front and just plain white on back.
CD itself has the Channel One address at the top and says its distributed bt Hitbound mfr, 89 N 4th Street Brooklyn NY.
Has the number JJ-020 on CD
By the way, I picked it up for £8.99 from System Records about 2 years ago and it had been on their site several times since then.
